DHT stands for dihydrotestosterone, an androgen your body makes from testosterone. It's a more potent androgen than testosterone itself — it binds the androgen receptor more strongly — and it drives much of testosterone's effect on libido, erections, and body hair. It's also involved in scalp hair loss and prostate growth.

Testosterone is the primary circulating androgen; DHT is a downstream metabolite made from it and is several times more potent at the androgen receptor. Testosterone can also aromatize to estradiol, but DHT cannot, so it produces "pure" androgenic effects. In practice, testosterone drives muscle and overall androgen levels while DHT concentrates its effects on libido, skin, hair, and the prostate.

The enzyme 5-alpha reductase converts testosterone into DHT, mainly in the skin, scalp, prostate, and genital tissue. Drugs like finasteride and dutasteride block this enzyme, which is how they lower DHT to treat hair loss or an enlarged prostate. Topical testosterone applied to the skin produces much more DHT than injections because it passes through skin rich in this enzyme.

The adult male reference range is roughly 30–85 ng/dL. On TRT, DHT typically rises in proportion to testosterone — often two to three times higher — and topical gels or creams push it higher still than injections. An elevated DHT on TRT is usually expected rather than a problem in itself.
