Ferritin is the body's iron storage protein. It's the best single measure of total body iron stores and also an acute-phase reactant.

What does high Ferritin mean?

  • +Iron overload (hemochromatosis), inflammation, fatty liver, heavy red meat intake.

What does low Ferritin mean?

  • +Iron deficiency.

TRT increases red blood cell production, which can deplete iron stores over time even without obvious anemia. Persistently high ferritin on TRT may indicate hemochromatosis — a common reason men feel exhausted despite good T levels, and frequently the actual reason their hematocrit is so high.

Only if you're deficient. TRT raises hematocrit via erythropoietin, not iron — extra iron without deficiency just adds oxidative stress. Donating blood for high HCT actually helps men with iron overload too.
