SHBG is the lever most men on TRT need to actually move to feel right. High SHBG means your free T is lower than your total T suggests — splitting your weekly dose into 2–3 smaller injections helps. Low SHBG means free T runs disproportionately high relative to total T, and you may need a lower total weekly dose than guidelines suggest. SHBG tends to drop with longer-acting esters and rise with daily / EOD dosing — sometimes counterintuitively.

SHBG carries sex hormones (testosterone and estradiol), while other transport proteins each carry different hormones — thyroxine-binding globulin carries thyroid hormone and corticosteroid-binding globulin carries cortisol. They're separate proteins with separate reference ranges, so a thyroid or cortisol panel won't tell you anything about your SHBG. On a hormone panel, SHBG is the one relevant to testosterone.
