/01What does TSAT mean on an iron panel?+

TSAT is the medical abbreviation for transferrin saturation. It's the percentage of your iron-transport protein (transferrin) that's actually carrying iron at a given moment. On an iron panel it's one of the most useful single numbers for telling iron deficiency apart from iron overload, which is why it's often the value clinicians look at first.

Transferrin saturation is a measure of how much of your blood's iron-carrying capacity is currently filled with iron, expressed as a percentage. Transferrin is the protein that ferries iron around the body; if only a small fraction is loaded, that suggests too little iron, and if most of it is loaded, that suggests too much. It gives a snapshot of iron availability that a single serum iron reading can't.

TSAT is calculated as serum iron divided by total iron-binding capacity (TIBC), multiplied by 100 to get a percentage: TSAT = serum iron / TIBC × 100. Serum iron measures how much iron is circulating, TIBC reflects the total capacity of transferrin to carry it, and the ratio tells you how "full" that transport system is. This is why TSAT, serum iron, and TIBC are usually reported together.

A typical reference range is roughly 20–50% for adult men, with slight variation by lab. Combined with a high ferritin, it's a standard screen for hemochromatosis, an inherited iron-overload condition, and often prompts genetic testing for HFE mutations, especially in men of Northern European descent. High TSAT can also occur with recent iron supplementation or hemolysis, so context matters.

A low TSAT — typically below about 20% — usually points to iron deficiency: there's not enough iron to fill the available transport capacity. It can also be depressed by chronic inflammation. Low TSAT is often the earliest iron panel change to appear before ferritin and hemoglobin fall, which makes it a useful early warning sign.

They describe different parts of iron status: serum iron is what's circulating now, transferrin (via TIBC) is the transport capacity, TSAT is how full that transport is as a percentage, and ferritin reflects your stored iron. Reading them together separates iron deficiency from overload far better than any one alone. This matters on TRT because men doing regular therapeutic phlebotomy or blood donation to manage a high hematocrit can steadily deplete iron and ferritin over time — TSAT helps catch that developing deficiency early so you can adjust with your physician.
